Pembroke Pines Community Health Assessment
Plan
To implement the intervention, the nursing practitioners and community partners involved in the nursing writing services will identify social centers dominated by the STD-risk demographic aged 21-35. The targeted social centers will include churches, local gyms, local markets, and institutions of higher learning situated in Pembroke Pines. Campaigns lobbying for STD testing will be launched in all the identified social centers and appointed speakers will teach the audience the benefits of screening for the diseases. Also, pamphlets with comprehensive information on STD infection, risk, and management will be offered for free to the targeted population.
Youths aged 21-35 who would like to get tested will register themselves with appointed representatives of Planned Parenthood – Pembroke Pines Health Center and Family Medical Center and book appointments for STD screening. Ultimately, all tested youths will be offered an armband as a symbol of their support for STD testing and counseling. Two community partners will be asked to participate in the STD testing and counseling intervention implementation. The partners are the Planned Parenthood – Pembroke Pines Health Center as well as Family Medical Center. The stated partners will not decline the invitation because they already offer HIV/STD testing and counseling services to the public members of Pembroke.
Two websites will be referenced as sources of support for the planned intervention. These will include the Project Ark website, which is focused on HIV/STD Counseling and Testing Family Medical Center website. The latter site belongs to the Family Medical Center, the healthcare institution based in Pembroke Pines. The two partners have resources that will aid in the implementation of the intervention. The resources will be two transportation trucks, free printed pamphlets on HIV/STD testing and counseling, and portable loudspeakers to be used when executing the intervention campaign.
The community partners will be ideal for the proposed intervention since they have been testing and counseling for HIV and STDs in Pembroke Pines. As such, they will have vast information on how STDs are transmitted among the youth, and any emerging trends in STD testing, treatment, management, and prevention. In addition, the two community partners will have physical resources like informative pamphlets on STDs that can be offered to the targeted demographic for free. The pamphlets will make it easy for intervention implementers to convince the youths aged 21-35 to seek STD testing and counseling. Also, the community partners will have two trucks that will be used for transportation.
Intervention
There is evidence-based support for the effectiveness of STD testing and counseling in management and prevention. Research by Hoover et al. found that STD clinics play a crucial role in STD prevention. Also, the clinics are preferred by most patients because they are convenient, provide expert care, and offer services at low cost. Whitlock et al. research also cited STD testing and counseling to be a primary intervention that can be employed to lessen the growing global rate of STD infection among young people. Also, research by Lanier et al. validated that HIV/STD testing and counseling is an effective intervention that can help prevent transmissions. Also, Lanier et al. argued that through early screening, patients with STDs can seek treatment before their conditions reach a critical stage.
The gatekeepers of the targeted social groups will be religious leaders, school administrators, gym instructors, and parents with children aged 21-35. The stated gatekeepers will be ideal since they have direct contact with the targeted population. To implement the intervention permits to conduct public rallies and campaigns on STD testing and counseling will be obtained from the Pembroke Pines Health Department offices. Also, consent to implement the intervention will be obtained from all the institutions of higher learning, churches, and gyms where the campaigns will be undertaken. Signed participation consent will also be attained from the two community partners prior to starting the intervention. Lastly, the campaigns will be started on December 31, 2018, and continue every day for six months except on weekends.
Evaluation
Short-Term Goals
1. Specific – make 60% of youths aged 21-35 go for STDs counseling and testing.
2. Measurable – success will be recorded if the two community partners participating in the plan record a statistical increment of 50% in the number of youths attending STD testing and counseling sessions in two months.
3. Attainable – to convince the two identified community partners to offer their help with the transportation trucks for plan implementation over the next six months.
4. Realistic – the intervention will have reduced the rate of STDs spreading among the youths by 20% in six months.
5. Time – In three months, the interventionists will have visited at least 10 institutions of higher learning, 20 churches, and 30 gyms as part of the intervention implementation plan.
Long-Term Goals
1. Specific – reduce the level of STDs in Pembroke Pines.
2. Measurable – success will be portrayed by the decline in the number of youths aged 21-35 who have STDs.
3. Attainable – the goal of the plan will be attained since transport will be offered by the two local community partners to join in the intervention implementation.
4. Realistic – the plan will attain significant results in 12 months.
5. Time – in six months, the interventionist will have visited enough schools, churches, and gyms to record a significant decline in the number of diagnosed cases of STDs among the targeted demographic in Pembroke Pines.
Key performance indicators (KPIs) will be adopted and used on all the interventionists to assess their performance levels in convincing the targeted youths to seek STD testing and counseling. The outcome criteria that will determine the effectiveness of the intervention will be a record of increase in the number of youths aged 21-35 visiting health clinics in Pembroke Pines for STD testing and counseling. Also, the intervention plan will allow clients’ involvement through;
i. Allowing the clients to ask questions about the procedure for STD testing
ii. Permit the clients to say what they feel about STD testing and counseling prior to seeking the intervention.
The intervention will have three distinct prevention levels. Primary prevention will be focused on training the clients on how to prevent themselves from STD infection by using contraceptives. Secondary-level prevention measures will be steps that can be taken by infected clients to allow early diagnosis and treatment for STDs. Lastly, tertiary prevention measures will be focused on ensuring that clients avoid re-infection after being sick with STDs for a long time and being treated in hospitals. Lastly, the intervention plan will advance the knowledge of the members of the community by enlightening them on the different STDs that they can contract. Also, the plan will educate the community on how to prevent, treat, and manage STDs in the event they have been exposed to the ailment.
